IELTS Test Report Form Third Party Pick Up Authorization

 

Allow another person to pick up your results for you using this form.

 

1. The test-taker should submit this form to us before pick-up.

 

2. The authorized pick-up person must have a copy of the test-taker's test day ID (passport or PR card).

 

3. The authorized person presents their own original ID (driver's licence, passport, PR card, refugee protection claimant document, Secure Certificate of Indian Status card) at the test centre office on the results release date.

I authorize the person above to receive my Test Report Form for the IELTS test I wrote. They will bring a copy of my ID document used to register for my IELTS test and their own ID.
